A Case of Exfoliative Dermatitis During Biologics Therapy (Adalimumab and Guselkumab) in Severe Psoriasis

Abstract

|
Exfoliative dermatitis is an inflammatory disease which is accompanied by erythema and scaling on localized or generalized distribution caused by various etiologic factors. We present a case of 46-year-old woman with severe psoriasis who developed exfoliative dermatitis during treatment of biologics known as adalimumab and guselkumab. Five months after the treatment of adalimumab, she experienced the aggravation of psoriasis, and then developed exfoliative dermatitis. The authors changed her biologic agent from adalimumab to ustekinumab, ustekinumab to guselkumab because of unsatisfied therapeutic effect. After guselkumab treatment, exfoliative dermatitis developed again at 11 months of guselkumab treatment. Since then, she has been treated with guselkumab on schedule, and exfoliative dermatitis did not recur. Exfoliative dermatitis developed during biological treatment is known to be rare, and there have been no reported cases during the treatment of guselkumab, so we report it with a review of the literatures.
